In addition, the science journalist recognized the achievements of this year&#8217;s TOP 100 companies at a joint online conference. The occasion was the TOP 100 final, which was originally planned as a face-to-face event. Ranga Yogeshwar is mentoring the innovation competition, which is being held for the 28th time. &#8220;True innovators are constantly changing, reinventing themselves and coming up with surprising solutions,&#8221; Yogeshwar said of the achievements of the TOP 100 companies. &#8220;During the Corona crisis, for example, they identified opportunities and came up with innovative processes in record time.&#8221; BioVariance was particularly impressive in the competition in the category &#8220;Innovation-promoting top management&#8221; in the size category up to 50 employees. The company&#8217;s innovation management is rated &#8220;A+&#8221; overall (on a scale of A to G). The &#8220;A&#8221; rating is awarded to companies that have unusually professional innovation management, even by international standards. Interdisciplinary work is also part of BioVariance&#8217;s innovation strategy. The team is composed of biologists, data analysts and software developers, among others, and aims to find innovative solutions for precision medicine in the healthcare, biotechnology and pharmaceutical sectors. In cooperation with physicians, the company monitors a patient&#8217;s molecular characteristics over a period of time to predict the efficacy, side effects and interactions of drugs and to determine the appropriate medication and dosage on an individual basis. Analysts develop cutting-edge technologies for oncologists and use scientific research to help them select the best possible therapies for their patients. &#8220;Regardless of which specialist area an employee belongs to at our company, we place a lot of value on everyone contributing and developing their ideas,&#8221; says Managing Director Dr. Josef Scheiber, explaining the company&#8217;s strategy. 										<content:encoded><![CDATA[<p>Within a few weeks, BioVariance GmbH established  a new lab for PCR diagnostics in the former hospital in Waldsassen in February 2021. Up to 1000 samples per day have been tested for SARS-CoV-2 infection since the establishment of the lab. The unique feature: After the samples have arrived in the lab, the processing and analysis of the samples usually needs less than 3 hours. A fast identification of newly infected people is essential for putting them in quarantine and thus avoiding further infections. In collaboration with the BRK, BioVariance made a great contribution to the rapidly decreasing incidence in the county of Tirschenreuth in spring 2021. But how can BioVariance reach this high pace? And how does PCR diagnostics actually work? These and further information about the workflow in the BioVariance lab can be found below.  </p>
<h3>qPCR diagnostics in the BioVariance lab</h3>
<p>The abbreviation „qPCR“ stands for “quantitative polymerase chain reaction”. SARS-CoV-2 is a so-called RNA virus, that means that its genome consists of RNA and not of DNA like in animals or humans. An organism can be clearly identified by analyzing its genome, which is the main point of the PCR analysis. A pharyngeal sample contains RNA in the form of potential viruses as well as DNA in the form of human mucosal cells. Therefore, the contained RNA is isolated and purified before the actual PCR. This RNA is then converted to DNA using the enzyme reverse transcriptase, because the PCR can only work with DNA. After this process the actual PCR starts: The process being called “amplification”, characteristic segments of the produced DNA are multiplied using the enzyme polymerase. BioVariance targets 3 specific DNA segments, while 2 of them belong to genes found in SARS-CoV-2: The RdRP gene, which exists only in SARS-CoV-2 specifically. And the E gene, which occurs generally in the subgenus <em>Sarbecovirus</em>, whereto also SARS-CoV-2 belongs. The evidence of the RdRP gene alone would be enough to identify a sample as positive, but BioVariance presupposes the detection of both virus genes for the generation of a positive report to avoid wrong-positive results. The third targeted segment, the RNAse P gene, is a human gene, which serves as an internal control factor for checking whether the sample contains enough biological material to avoid wrong-negative results.</p>
<h3>Evaluation of the PCR analysis</h3>

<p>  But how can we check whether the amplification is successfull? Using a special PCR cycler (see Fig. 1), fluorescence markers are embedded into the multiplied DNA sequences during the PCR. Every target sequence is associated with another fluorescence colour. The more DNA is produced by the polymerase, the stronger is the fluorescence of the sample in the real time measurement. For generating an unequivocal result – no matter if negative or positive – the fluorescence signal of the human RNAse P gene must be sufficient. If there is additional fluorescence of both virus genes, the sample will be marked positive. The Ct value serves as a scale unit for the number of existing genes. It describes the number of DNA multiplication steps being necessary to differentiate the fluorescence signal clearly from the so-called background noise and to generate a reliable test signal. The lower the Ct value is, the higher is the number of existing target genes and thus the number of virus particles. Conversely, a high Ct value shows a low concentration of virus particles. During the PCR, the fluorescence signal arises until it peaks after about 1 hour. After this peak it flattens out because the added components for DNA multiplication are used up. This process creates the typical sigmoidal curve of the fluorescence (see Fig. 2).  </p>

<p>  But even the best procedure is subjected to limitations. In about 1 % of cases the result of the PCR is not unequivocal, because the sample material is insufficient or the fluorescence signal is too low, for example. In these cases, the PCR is repeated to gain a clear result.</p>
<h3>Identification of virus variants using qPCR</h3>
<p>In the BioVariance lab all positive samples additionally undergo a special variant PCR analysis to identify the respective SARS-CoV-2 variant, targeting several SARS-CoV-2 specific gene segments containing the yet known mutations. These analyses revealed that in the last weeks the former virus, the so-called wildtype, was gradually replaced by the British variant B.1.1.7, today called “Alpha“, in the county of Tirschenreuth. Furthermore, positive samples are also sent to a partner lab of BioVariance for further variant analyses, so that new and yet unknown mutations can be identified.</p>
<h3>Screening of schools using pooled gargle samples</h3>
<p>In May 2021, BioVariance has introduced a new pooling procedure, with several schools in the county of Tirschenreuth participating in the context of a study. This procedure does not need the pupils to take a nose or pharyngeal sample, but only to gargle with some tap water in the morning at home. At school, these gargle samples are “pooled” per class, that means mixed in one sample cup (see Fig. 3). Thus, only one PCR analysis has to be done for each class. Only if a pool sample is positive, the gargle samples of the referring pupils will be analyzed individually. This process allows great savings of lab resources while maintaining a high quality of results.  </p>

<h3>Quality management in the BioVariance lab</h3>
<p>BioVariance regularly faces external evaluations to ensure the quality and safety of their PCR analyses. For these so-called ring trials, the lab receives several unknown samples from a central office. Only if all samples are identified correctly as negative or positive, the lab will pass the trial and gain a certificate. In May 2021, BioVariance has passed again the external test from the European Society for External Quality Assessment (ESfEQA). Further, all incoming samples are treated under sterile conditions and conforming with strict hygienic regulations in the BioVariance lab, avoiding the contamination of the workstations and a possible infection of employees (see Fig. 4).  </p>

<h3>Speed of PCR diagnostics in the BioVariance lab</h3>
<p>In Germany, it usually takes 24-48 hours until a tested person receives the PCR result. In the BioVariance lab, results can be produced in less than 3 hours after the samples arrived in the lab. So 99 % of the tested people receive their result at the same day. Two special components allow BioVariance to reach this speed: On the one hand, BioVariance uses an optimized procedure for the RNA extraction which guarantees high efficiency, precision and safety. On the other hand, the IT specialists of BioVariance developed a purpose-built software called <a href="/?p=5676">TubeLab</a> enabling the digital monitoring of the lab processes, which clearly accelerated and facilitated the handling of samples. Using TubeLab for a transparent tracing and documentation, the BioVariance lab meets the highest requirements. Besides, the partner lab doing the further sequencing of the positive samples also uses TubeLab for monitoring their PCR analyses since April 2020. BioVariance also increased its personnel and expanded the lab team by 2 technical lab employees and 2 student assistants from the scientific area.</p>
<h3>Strategic success in Tirschenreuth</h3>
<p>With the help of the administrative district office and the health office, the administrative barriers for the establishment of the diagnostic lab as from 26.02.2021 in Waldsassen could be removed within a very short time. Short ways between the test station and the lab are indispensable for rapid testing. In the best case, both stations are located in the same building like in Waldsassen. And thanks to the BRK, 4 permanent and several mobile test stations could be established. More than 40.000 pharyngeal samples from people living in the county of Tirschenreuth have been analyzed since the commissioning of the lab. Using the highly efficient PCR diagnostics, infected people could be identified and quarantined within a few hours. This fact also had a strong positive influence on the local incidence. In February 2021 when the lab was commissioned, the 7-day-incidence was more than 350 infections per 100.000 inhabitants in the county of Tirschenreuth. Now, 15 weeks later, the region is the first county in Bavaria to show a 7-day-incidence of 0. The effective collaboration of the administrative district office, the health office, the BRK and the BioVariance lab as well as the consistent endurance of the population made it possible to signficantly reduce new infections and to finally conquer the high incidence in the county of Tirschenreuth.</p>
<h3>Future orientation of the BioVariance lab</h3>
<p>BioVariance aims to expand the application possibilities in the lab and to integrate the diagnostics of cancer diseases into their portfolio, additionally to the existing diagnostics of infectious diseases. The company has always focused on the personalization of cancer diseases by sequencing tumors and using the homemade software <a href="/?p=5694">OncoVariant</a>. This pioneering step allows the selection of the most suitable therapy for cancer patients individually to achieve the greatest possible effectiveness. <p>In cooperation with the Administrative District Office, the Health Department, the BRK and some schools in the county of Tirschenreuth, the BioVariance laboratory recently started a pilot project for PCR testing of pupils and their teachers. A new gargle-method has been developed to enable soon the area-wide SARS-CoV-2 testing of all pupils twice a week. .</p>
<p>Here, the pupils gargle with a small amount of tap water at home and divide this sample fifty-fifty into two tubes. At school, the first half of the sample is then poured in a common cup for each class, the second tube is stored individually. This “pool” of several samples is tested for SARS-CoV-2 in the BioVariance laboratory using the established PCR method. The PCR is highly sensitive that even a single positive pupil can be detected in a pool of otherwise negative samples. If the pool result is positive, the corresponding school must be informed immediately. Then the second sample tubes of the class with the positive pool test are collected and tested individually by PCR on the same day to identify the positive person.<span data-ccp-props="{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559739&quot;:160,&quot;335559740&quot;:259}"> </span></p>
<p><span data-contrast="auto">By now, eight schools are participating in this pilot project. More schools are supposed to follow in the next days and weeks. Further, the gargle method is currently also examined for the possible use in day-care centers. </span></p>

<p><img loading="lazy" decoding="async" class="wp-image-5774 alignleft" src="https://panzerneumann.de/biovariance-old/wp-content/uploads/2021/02/logo-_3d.jpg" alt="" width="445" height="314" />On behalf of compamedia, the organizer of the TOP 100 innovation competition, Prof. Dr. Nikolaus Franke and his team examined the innovative power of BioVariance based on 120 test criteria in 5 categories: innovation promoting management, innovative climate, innovative processes and organization, outward orientation / open innovation, and innovative success. The examination addresses the questions, whether innovation is the result of organized processes or a random product, that means the repeatability of innovative performances, and whether and how the corresponding solutions succeed on the market (more information about the test criteria on <a href="http://www.top100.de/pruefkriterien">www.top100.de/pruefkriterien</a>). Due to the current situation another category was included in the examination: the reaction of the company to the corona crisis.</p>

										<content:encoded><![CDATA[<p><h3>The Goal &#8211; Advance Personalized Medicine</h3>
<p>BioVariance GmbH focuses together with a global player from the pharmaceutical industry on advancing research in personalized medicine. Due to the outstanding cooperation and the excellent expertise of both partners, a new categorization of tumor indications will be available for the pharma and healthcare sector. The new categorization based on the individual characteristics of the tumor tissue shall provide a deeper understanding of disease mechanisms and further allow a more individual diagnosis and a more targeted and personalized therapy for affected patients prospectively.</p>
<h3>Challenges</h3>
<p>The rapid development of technologies for the biomedical and pharmaceutical research enable the generation of complex interdisciplinary data sets. Within the scope of this project, next-generation sequencing (NGS) and innovative optical measurement techniques were applied to characterize tissue sections to newly categorize the tumor indication of interest. Due to our broad expertise in Big Data related challenges, such as data processing, exploratory statistics and machine learning, the BioVariance GmbH was assigned to perform preprocessing of the datasets and to develop and evaluate the clustering models. To make both NGS and tissue characteristics data sets comparable and to correct for a possible bias caused by the new optical measurement technique, normalization of each data set was mandatory. The challenge was to find an appropriate normalization strategy that fulfilled both requirements. In addition, normalization should not produce spurious correlations within the data. Therefore, several normalization strategies were selected and tested. To ensure the absence of spurious correlation within the normalized data, the coefficient of variation <sup>[1]</sup> and partial correlations were determined. Finally, the best normalization strategy was chosen taking all three criteria (comparability of data, correction of bias, absence of spurious correlation) into account. Thus, the data scientists ensured, that the linear correlations between two variables of the data set (visualized by the scatterplot matrix) were not due to spurious correlations. The scatterplot matrix, however, did not show any clusters in the pairwise comparison of variables (in contrast to the scatterplot matrix from test data presented in Fig. 1). Consequently, the next step comprised a principal component analysis (PCA). For this purpose, the normalized tissue characteristics required a Box-Cox transformation, since none of these variables followed a normal distribution.</p>

<h3>The road to success</h3>
<p>Within the scope of this project, the team of data analysts at the BioVariance GmbH relied on descriptive statistics including state-of-the-art algorithms and exploratory data analysis to uncover differences, correlations and clusters in the data.</p>
<p>No clusters were observable in the scatterplot matrices. Consequently, a PCA was enriched by the Marchenko-Pastur distribution to find the discriminating factors. PCA reduces the number of variables in a data set by searching linear combinations, that describe the highest degree of variance &#8211; the so-called principal components (PCs). These PCs are the new variables that have the potential to visualize inherent clusters. The first PC describes the largest ratio of variance in the data, the second PC the second largest, etc. They construct an orthogonal coordinate system, of which the axes point into the direction of highest variance. Plotting the first two or three PCs is often sufficient to visualize clusters.<sup>[2]</sup> By means of the Marchenko-Pastur distribution the number of PCs carrying discriminating information was determined.<sup>[3]</sup> In this case, the first PC was identified as the only significant factor.</p>
<p>To confirm the visual result from the PCA, several clustering algorithms (hierarchical clustering using Manhattan distance, k-Means and PAM algorithms as well as dbscan) were</p>
<p>performed. Applying two parameters (e.g. the so-called silhouette-width) that describe the quality of cluster assignment for each patient, the most differentiated clusters were identified.</p>
<p>These newly discovered groups could be visualized and reinsured by labeling them in the PCA-Biplot (see Fig.2).</p>

<p>An established categorization which is based only on gene expression was compared to the new categorization. The results of this comparison were visualized by a heatmap representation. The two categorizations could be clearly distinguished from each other. In this study, the gene expression data of the tumors were applied additionally to identify biochemical processes responsible for the tissue characteristics.</p>
<p>The results from this successful cooperation were published in a high impact cancer research journal. They will strongly support the development of personalized therapy strategies.</p>

To answer research questions adequately, it is of utmost importance to successfully uncover hidden patterns and correlations in these data sets. Thus, the challenge in science today is mostly not the generation of sufficient data sets but their bioinformatic analysis. Consequently, it is time to provide a platform that allows to manage and analyze Big Data efficiently. Therefore, BioVariance GmbH developed BioCore Studio.</p>
<h2><strong>What is BioCore Studio?</strong></h2>
<p>BioCore Studio is a web-based software for the assembly, automation and parallelization of data analysis workflows, so-called pipelines (see Fig. 1). It was built to assist in computation-intensive biomedical research.</p>

<p>The platform comprises tool packages that enable the user to intuitively design tailored analysis pipelines by drag and drop. There is no programming knowledge required. Additionally, BioCore Studio is suitable for all data types.</p>
<p>Visualization of the process in real time and insights into results and output of each step guarantee a transparent workflow. Regularly occurring tasks can be scheduled or started immediately. Due to its innovative parallelization and automatization technologies BioCore Studio enables time and cost savings as well as an optimal utilization of technical resources.</p>
<p>BioCore Studio is flexibly scalable in terms of computing power and adapts to individual needs. Huge amounts of data can be processed quickly and safely. Thanks to the Kubernetes cluster, available computing capacities are used efficiently.</p>
<h2><strong>The Product in Action</strong></h2>
<p>The BioVariance GmbH employs BioCore Studio to analyze high-throughput NGS data for its clients in the pharmaceutical and medical industry (see Fig. 2). For this purpose, ready-to-use pipelines are implemented. These are continuously expanded and optimized by the BioVariance GmbH with regards to new biomedical trends.</p>

										<content:encoded><![CDATA[<p>The global SARS-CoV-2 pandemic is an emerging, rapidly evolving situation. Therefore, increasing the test capacity to detect a virus infection is of utmost importance. BioVariance GmbH helped to fortify the regulatory submission proposal of a globally acting <em>in-vitro</em> diagnostics company, to receive FDA (Food and Drug Administration) approval for a novel and rapid COVID-19 PCR test. This was achieved in record time by employing computer aided (<em>in-silico</em>) validation methods, and the dedication of our team of Data Specialists. Within the scope of this project, two main objectives were confirmed:</p>
<h2>Primer and Probe Sensitivity</h2>
<p>To confirm primer and probe sensitivity, alignments of the oligonucleotide sequences and all publicly available SARS-CoV-2 sequences were performed. Hits were prioritized by scoring criteria such as e-value, and mismatches were assessed and evaluated. Furthermore, the hits were further filtered for a potential formation of PCR products.</p>

<p>To predict potential cross-reactivity of the oligonucleotides, sequence homologies with the human genome, other coronaviruses and respiratory related pathogens were detected. This step was essential for ruling out false positive PCR results.</p>

										<content:encoded><![CDATA[<p>Waldsassen &#8211; BioVariance finishes the development of TubeLab, a software for the organisation of multi-level laboratory processes. To ensure an effective performance of the workflow, TubeLab was developed in cooperation with a renowned diagnostics laboratory. In this lab, TubeLab also expedites SARS-CoV-2 coronavirus tests under the name of COVIDetect. Thus, hundreds of patient samples can be processed quickly, safely and trackably every day. We at BioVariance GmbH are proud to contribute to the containment of the pandemic.</p>
<h2>What are TubeLab&#8217;s features?</h2>
<p>TubeLab digitalizes sample handling and tracking, process tracking as well as the evaluation of signals and quality. As a result, the overall duration of the coronavirus test workflow &#8211; from sample scan to the transmission of results &#8211;  can be reduced significantly.</p>
<p>The software visualizes every work step in real time and thus documents who executes a certain work step at a certain time. Furthermore, TubeLab validates thresholds by request and informs the user when, for example, the quality control fails the predefined criteria (see Fig.1 ). These features allow a quick and safe processing and facilitate the organisation and control of the workflow.</p>
<p>Another fundamental benefit is TubeLab&#8217;s intuitive operability and flexibility.  The software can be installed on every lab-own server with a granted access to the intranet. Besides, the high scalability allows the processing of an unlimited amount of data. Data protection is guaranteed in every single work step. Thus, large amounts of data can be processed, stored and transmitted in a safe and fast manner.</p>

										<content:encoded><![CDATA[<p>Waldsassen &#8211; BioVariance has finished the development of the platform <a href="https://oncovariant.com/">OncoVariant</a> for the interpretation of genetic variants in cancer patients. Thereby, the crucial search for suitable drugs is simplified and accelerated significantly.</p>

<p>Nowadays, the manual search for the optimal medication is still a labor-intensive and time-consuming process for oncologists. Every patient and tumor shows different reactions to drugs due to individual genetic conditions. At the same time, the amount of data the physician has to examine has grown exponentially in the last years, resulting in a higher expenditure of time and work.</p>
<p>At this point, BioVariance offers state-of-the-art automatization and parallelization techniques: The web-based and user-friendly application OncoVariant enables the oncologist to consult the worldwide existing literature for the selection of the optimal treatment. All publicly available databases containing information about gene-drug correlations are included in the query. Patient’s genetic variations are automatically compared to the profound knowledge in these databases to identify individual treatment options.</p>
<p>The oncologist receives a final report with a summary of all information found by OncoVariant. This report shows which genetic variants have to be considered for the treatment of the cancer patient and which medication promises the greatest success for this patient.</p>

										<content:encoded><![CDATA[<p><h3>The facts</h3>
<p>„Colorectal carcinoma“ means several malignant tumor types in the colon or rectum. Malignant tumors rarely occur in other intestinal areas like small intestine or caecum. <sup>[1]</sup></p>
<p>Colorectal cancer (CRC) had a low incidence rate in 1950. However, it became a predominant cancer type and now accounts for about 10% of cancer-related mortality in western countries.<sup>[1,2]</sup> CRC is the second most common cancer type in men and women in Germany. It ranks third behind lung and breast cancer worldwide. In Germany, about 26.000 women and 32.300 men developed CRC in 2016, while 11.400 women and 13.400 men died from it. More than half of the patients developing CRC had an age of 70 years and above. Only 10 % of the new cases occur before the age of 55. <sup>[2]</sup> The lifetime risk of developing CRC is about 6 %, with chronic inflammatory bowel diseases and other important factors like unhealthy diet or lack of exercise raising the cancer risk. Inheritable genetic mutations cause 3-5 % of all CRC cases, especially colon cancer. Affected people have a very high risk of developing cancer in the early age of 20-40 years. <sup>[1]</sup></p>
<p>In the year 2018, the World Health Organization counted about 1,8 Mio new CRC cases and 880.000 deaths. <sup>[3,4]</sup> More than 50 % of the new cases were registered in Asia (see Fig. 1). The global incidence rate is estimated to increase up to 3 Mio until 2040. <sup>[3]</sup></p>

<p>As described above, men are stronger affected by CRCs than women. <sup>[2,3,6]</sup> Additionally, cancer cases strongly increased in the developed countries due to the ageing population in the recent years (see Fig. 2). <sup>[3]</sup></p>

<h3>Sex differences</h3>
<p>Although women have a lower incidence of CRC than men, right-sided CRCs occur more frequently in women compared to men, whereas left-sided CRCs have an equal frequency between both sexes. Several evidences demonstrate that the rate of right-sided CRC cases is strikingly higher in women than in men (61.7% vs 38.3%), while only slightly more left-sided CRC cases are observed in women than men (52.1% vs 47.8%). The reason for this sex difference is still not proven and of concern for women, due to the association of right-sided CRCs with the poorest clinical outcomes among all CRC patients. <sup>[5]</sup></p>
<p>Studies revealed that metabolite rates in the glycolysis pathway, <span class="st">pentose phosphate</span> pathway, carnitine shuttle metabolism, asparagine synthesis, methionine metabolism and the polyamine synthesis pathway showed sex-related differences when measured in tumor samples compared to normal samples. Tumors from women and men also used different metabolic intermediates between the sexes for energy production to support cell growth. <sup>[5]</sup> Thus, modern CRC therapy approaches should ideally consider not only common molecular information, but also details of sex-specific mechanisms.</p>
<p>Further studies also indicate that the gut microbiota is partly correlated with the tumor development as well as emerging drug resistance. <sup>[4,6]</sup></p>
<h3>The upgrade of personalized colorectal cancer treatment</h3>
<p>Personalizing cancer treatment is still challenging. No expert believes in finding a magic cure against CRC nowadays. However, therapeutic approaches based on patient’s genetic variations provide the most promising chances for patients. Numerous genetic mutations are already associated with risk of CRC. <sup>[7]</sup> But the manual search for gene variants and appropriate drugs in cancer therapy is very labour intensive and time consuming, while the number of new medical insights gained from scientific research continuously increases. Further, economic acting must be brought into accordance with patient welfare.</p>
<p>Therefore, BioVariance has now developed the web-based platform <a href="/?p=5694">OncoVariant</a> to identify the most suitable medication for patients suffering from colorectal, breast or prostate cancer. State-of-the-art automatization and parallelization techniques are combined for comparing patient’s gene variants with profound knowledge from worldwide databases regarding treatment options. The final report contains information about patient’s variants and appropriate clinical evidence as well as information on the proposed drugs and their interactions. The examination of numerous high-quality databases ensures the high value of the proposed therapy options.
